It's usually a one-time payment for the policy itself, but they also have options for hosting and update notifications that might be annual. You can just buy the document though.

Yeah, I just paid once for my privacy policy and downloaded it. You can pay extra for them to host it and get updates if you want, but it's not mandatory.
